Welcome to Duke Prospective Health
Duke Prospective Health can help you improve your health and well-being by evaluating your current health status and helping you to create a personal health plan. You'll be connected to resources to help you meet your health goals. The program focuses on preventive health services and especially on cardiovascular disease, hypertension, and diabetes. This is a voluntary and confidential program.
Participate in the program and qualify for a prize package worth $150! All members who complete a 2010 Health Risk Assessment by the end of February will also be entered into a drawing to win an iPod Touch.
Members of Duke Select and Duke Basic are automatically enrolled in Duke Prospective Health. They can begin their active participation in the program by completing a simple health questionnaire called a Health Risk Assessment, or HRA, available
online. Employees and covered dependents 18 and older are eligible to participate.
