Qualified members who submit a Health Risk Assessment and who choose to participate in the DPH Care Management Program can receive up to $150 toward your mail-order prescription drug copay for working with a Care Manager every other week for four months, establishing personal health goals, and visiting your Primary Care Physician. Additionally, you can earn $50 toward your mail-order prescription copay every 60 days with continued contact with your care manager.

Qualify for monthly drawings worth $150

Any participant who completes a Health Risk Assessment will be entered in monthly prize drawings. Winners will be able to choose prize packages relating to physical fitness, nutrition, and stress management. These prizes require customization and will be deliverable within 30 days. Current prize packages are:
 

• Working it Out: includes a three to six month membership to a local gym and a gift card to Dick's Sporting Goods
• Walk this Way: includes an iPod shuffle and a gift card to Dick's Sporting Goods
• Oh, my Aching: includes a gift certificate to a local day spa
• Healthy Living: includes a gift certificate for $75 to Whole Foods, and a $75 gift certificate to Bed, Bath & Beyond.
