Care Management

Individualized care management services are available to qualifying participants. If you qualify, a Certified Care Manager will discuss options with you to help you create a workable plan specific to your health care needs.

This free benefit enables you to work one-on-one with a Care Manager, who will serve as a Duke Basic or Duke Select member’s central point of contact for the program to provide:

Clinically focused personalized health education
Integrated coordination of your physician’s care plan to improve compliance and success
Administration of earned awards and incentives
Appropriate referral and coaching services
Assistance with the creation of a personalized health plan
Supportive methods to help you reach the goals of your personal health plan and;
Assistance in tracking your progress to successfully meet your goals
