| Coaching Program Support Services |
Coaching services are available to qualifying participants. If you qualify, you will be contacted by a Care Manager. Coaching is designed to support participants in carrying out their Personal Health Plan goals. A Care Manager will ask you a series of questions to determine if you are ready to make the commitment it takes to work with a coach.
If you are ready, then you will be assigned a professional coach who will support you in meeting your goals.
What is coaching?
Coaching is for participants that are ready and motivated to make healthy changes, but would like to have support and resources to help them.
"Professional coaches are trained to listen and observe, to customize their approach to the individual client's needs, and to elicit solutions and strategies from the client. They believe that the client is naturally creative and resourceful and that the coach's job is to provide support to enhance the skills, resources, and creativity that the client already has. While the coach provides feedback and an objective perspective, the client is responsible for taking the steps to produce the results he or she desires. Coaching does not focus directly on relieving psychological pain or treating cognitive or emotional disorders."

How do I find out if I qualify?
If you qualify for coaching services, you will be contacted by a care manager.
What happens once I'm registered for coaching services?
Once you are registered in a coaching group, you will meet in person with your coach and group once to fine-tune your Personal Health Plan. Click here for more on the Personal Health Planning concept used by coaches.
After the initial in-person meeting, you will meet with your coach and group either in person or over the phone. In these sessions, the coach will lead you through a supportive meeting in which you are encouraged to talk about your progress on your goals as well as any challenges you are meeting as you work on your goals. The coach will direct you toward valuable resources and help you decide the best course of action for you.
In addition to the coaching you receive, you will have access to monthly seminars on different areas from the Wheel of Health. You will also have access to monthly skill-building workshops that focus on helping people change their lifestyles. Skill-building workshops include such topics as overcoming obstacles, relapse prevention, building healthy support networks, and effective communication skills.
