What’s Available for Me?
Duke Prospective Health offers a set of tools and services to allow you to develop and maintain your Personal Health Plan. The following items are available to Duke Prospective Health Participants:
A Health Risk Assessment (HRA) is a questionnaire that asks about your current lifestyle and habits including physical activity, nutrition and stress management. You can use the results of your Health Risk Assessment to help set goals for healthy actions in your Personal Health Plan.
Track your progress in meeting Personal Health Plan goals and track key health measures that are tied to you personal health goals.
Depending upon your health needs, you may qualify to work one-on-one with a Care Manager, who will serve as your central point of contact for the program, maintain regular telephone contact with you, and provide on-going assistance in reaching your goals.
Depending on your health needs, you may qualify to work in a group setting with a health coach to help build new skills and establish healthy behaviors for meeting your goals.
- Referral (For Qualifying Participants)
Depending on your health needs, you may qualify for an individual referral into programs to help you reach your health goals. Programs range from smoking cessation to weight management and are offered in person, electronically or telephonically.
All Duke Basic and Duke Select subscribers are eligible to participate in a monthly drawing for a $150 prize package by completing a Health Risk Assessment. Additional awards are available throughout the year for qualifying participants.
